When babies are born at Adventist Health Lodi Memorial, the medical center completes the birth certificate for the newborn and files it with the county where the baby is born. By law, the medical center cannot keep a copy of the birth certificate.

To obtain a certified copy of a San Joaquin County birth certificate, an application form must be completed and a sworn statement must be signed. Phone requests for copies of birth certificates are not accepted by the county. There is a $25 fee, and ID must be provided.

For children under two years old

Birth certificates may be picked up at the San Joaquin County Public Health Services office in Stockton. Or, you can use the application statement if needed (often an option for those who are not local and wish to receive it via mail).
